One-time enrollment fee of $129 for all new or inactive patients.
Details & Limitations: Adult plan is for patients 14 years of age and older. Child plan is for patients 13 years of age and under. If a patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost. The perio plan is strictly for preventive maintenance care. If the patient chooses to pay for treatment with CareCredit the discount will be voided.
Terms & Conditions: This is not dental insurance. Cannot be combined with other insurance or discount plans. Membership plan will auto-renew each year on the renewal date. Membership plan is specific to one person and cannot be transferred. Services will be provided at this location only. Covered services not used within the specified time period will not be carried over or transferred. Cancellation fee: If the patient cancels before the year is completed, the cancellation fee will be the lesser of 1) remaining payments on the plan for the year or 2) the value of services received to date.
